Does everyone need the custom map in order to play it in multiplayer? Or does only the host need the custom map?

Right now if only the host has the game the map will 'download' to the other players. Last I checked this was very slow to do it in this way, so if possible it would be best for everyone to download the map on their own - but that also depends on the level and the size, the host's upload speed, etc.

This is true.
The host has the level and the clients downloads it if they miss it. So the upload speed is mostly the bottleneck here.

Maybe we could implement some kind of option during the making of the campaign for the builder to host it via an FTP link or dropbox link - and then have it distribute from that link instead of the host? :D ;)

Just put a tiny address to your file in the map description ;)

That's a good idea, and for self capable people that would work. I'm just afraid of someone not getting it or because its not 'official' (in the sense that the game auto-loads it) that people will be afraid to use the URL. URLs can be dangerous these days :(
