Bug Reports / [Windows] Staircases + death causing second player to not respawn

Sorry for the poor title, it's just an awkward thing to try to explain.

This has happened to us three times and in two different ways. I hosted the server to play the campaign with one friend with the infinite lives crutch on.

• I was standing on the boss staircase in the 3rd act and he died. It brought us to the boss room yet he would not respawn. He respawned in the 4th act after I beat the boss.
• I was standing on the Floor 11 to Floor 12 staircase and he died. It brought us to the 12th floor yet he would not respawn. He did not respawn until the game was over.
• He was standing on the Floor 11 to Floor 12 staircase while I was elsewhere killing enemies. He died and it brought us to the 12th floor and he would not respawn. We quit at this point.

We could not get him to respawn. I tried going up and down different staircases and having him reconnect to the server.
