Need some help with 3 achievements

Would anyone be so kind to explain where or how to get the following achievements please?
  • The Heist
  • Harvesting
  • Genocide - Kill 250,000 enemies in the campaign

The two first are two easter eggs hidden in secret areas.
The last one is just killing 250k monsters.

Do I have to kill these monsters in one single campaign, or can I use multiple campaigns + both single / multiplayer?
For example, can I just repeat Floor 1 over and over again to get the achievement?

Yes, that should work and both MP and SP and not only in one campaign, over multiple.
