Where is Crystal Lich? (survival achievements)

I thought the Survival level is no longer in game, but these two achievements are still in available:

Crystal Lich
Kill the Crystal Lich in the survival level on medium difficulty without having any crutches enabled

Hard Crystal Lich
Kill the Crystal Lich in the survival level on hard difficulty without having any crutches enabled

I've finished the whole game and couldn't find it anywhere.
I was even following the 'all secrets' guide from Steam Community.
Any hints on how exactly to get there?

Ranger is the best strategy, if you survive to when he comes and battles you, you have won, the thing is that his aggro range is longer than his magic balls range, and the ranger can stand outside of Crystal Lich's ball range and still do damage :p

lol, I didn't know there is a 'proper' battle - I couldn't survive that long : (((
Does he has alot of HP? Should I focus on farming the crystals and invest in upgrades, or just DPS him instead?

Hmm, I don't remember, I think you should try and upgrade your damage as much as possible in that level, if your good enough to take as little damage as possible.

this is one achievement i dont think i will get. i couldnt beat him on easy  i ran into a problem though... perhaps it was just bad luck. after i got his health down to 75% and he started shooting a crap load of seeking energy balls, he was in a place where i could only approach from the sides, and it was a long distance to travel. and unfortunately i couldnt get close enough before i died to even hit him once. is there designated places that he goes durring certain periods of his life?

If you are using an Archer, you can keep shooting him from the distance and his spells won't reach you at all.
I think he teleports to different places only at the start of each phase (100% / 75% / 50% / 25%).
During each phase his movement is very limited - he keeps moving forwards and backwards (it can be tricky as you should always keep the distance from his spells and sometimes he stands too far away to be visible on the screen - you can still see him on the map though).

The goal is to stay as far from him as possible while still doing the damage.
If possible, position yourself to shoot him horizontally (left - right) instead of vertically - you will have much higher view range on the screen.
The "crap load of seeking energy balls" phase shouldn't be a problem at all - his spells still won't reach you - just keep the distance.
